This a single linear rail version for the bear y axis to avoid alignment issues. The model is based on the bear frame (https://guides.bear-lab.com/) and was inspired by the Switchwire y-axis mount (https://github.com/VoronDesign/Voron-Switchwire).

Putting it together is quite straight forward. The additional v-slot profile is to be placed in the center of the y-frame assembly and to be loosely fitted with the brackets. They are a bit finicky to get on, but a mic of hammer and t-nuts did the trick for me. Then the linear rail can be fitted with m3 bolts and finally the distance plate and y carriage. The alignment jig needs to be screwed to the outermost edges of the carriage and to the front 4020 v-slot to assure the carriage is level. Then the bracket can be fixed tightly. This needs to be done for the front and rear.
Additionally, an customized version of the belt holder and the Mk2/2.5 version of the motor mount need to be printer to assure belt alignment. The idler-assembly can be carried over.
I have included a step-file of my model, that includes slightly altered versions of the Voron Trident skirts (https://github.com/VoronDesign/Voron-Trident). Also bottom panels are included with holes for a Meanwell LRS 350-24 power supply to have it mounted below the print bed. There is also enough space for the rest of the electronics.
I do run my printer with a btt board and klipper, therefore I cannot tell if a Prusa printer will pass self-test. I'd need feedback on that.
