Summary
I was previously using Sobaldrick's https://www.thingiverse.com/thing:3191174 but upgraded to a Pi 4. Since I couldn't find one that worked for my as well as his design, I mixed up my own. I got the measurements for the front and side holes from Tomsc's design, https://www.thingiverse.com/thing:3464513/files and the side rail profiles from Sobaldrick's design. Basically projected the models onto sketch and then edited the sketches. I added a slide-together design so it could be printed upright with minimal supports. I used 3mmx6mm button heads for the RPi4, and 3mmx10mm for the fan.

base 2.5mm thread (holes are the same size as RPi4 so no need to drill out pcb)
base 2.5mm thread opposite rails (no side io ports)
lid 40mm fan-gpio-opposite rails (no side io ports)
fan plug 30mm -grill (.2mm clearance fit)
fan plug 40mm -grill (.2mm clearance fit)

printed without upper or bottom layers, triangular (2 lines, in cura), or fast honeycomb (S3d)..wish cura had honeycomb. You can print w/ top/bottom if you want, though. 4 to 6 outer layers. You may need to tweak the size of the fan/gpio plugs to get them to fit if you choose to use them.
