I added some double sided tape to hold it in place. One issue I do have is my PTFE cable likes to clack on the led lip, probably just some adjustment on my end. I completely rebuilt the model. I have adjusted a few things.

Dimensions minimally adjusted
Break-out cable entry at the front and back
20mm x 20mm hole pattern on the sides to enable attachments to be screwed on
12mm LED are possible
the LEDs can be attached with cable ties (holes in the model)
a lip in front of the LEDs so that you are not blinded
connection of the individual parts using a connector in the shape of a bone. This makes it easier to adjust the fit of the parts later
