Thanks to @elcubano for having this design to use. All I had were M3 screws and T-Nuts. I modified his design to use what I had and seems to work great.

--Print Info--
Printer with Eryone Clear PETG.
3 Parameters
30% Cubic Subdivision Infill
Normal Supports Everywhere (This created tiny supports just around the two horizontal holes which were easy to remove.)

Screws Used:
x2 M3-20mm screws for the raised top.
x2 M3-12mm screws on the lower back.
x2 M3-12mm screws with T-Nuts to secure it to the printer.

--Other Notes--
I attempted to keep it a similar height as the original mount by using a stack of cards. Took a while and tested it with shims, but eventually found that they were not needed. Used the original coupler and added an Anti-backlash kit to my existing Oldham coupler. (The coupler was getting worn and loose.)

The mods with freshly cleaned/correctly aligned rods works great.
